Pelvic presentation of the fetus occurs in 5-7% of full-term pregnancies and carries 3-4 times more complications than cephalic presentation. In most cases, it is an indication for cesarean section, especially when the fetal weight exceeds 3700 g. However, modern international clinical guidelines (WHO, FIGO) recommend considering the possibility of performing external obstetrical cephalic rotation, which reduces the risk of complications and increases the chances of natural childbirth.
This manipulation was successfully performed with the participation of the Department of Obstetrics and Gynecology of the Shupyk National Healthcare of Ukraine together with doctors of the Vyshgorod Central Regional Hospital. The procedure was performed by Professor of the Department Oleg Volodymyrovych Golyanovsky and Head of the Obstetrics and Gynecology Department Iryna Anatoliivna Gubar.
A 40-year-old patient with a full-term pregnancy (38-39 weeks) and diagnosed breech presentation of the fetus was hospitalized to the department. After a thorough examination, including ultrasound control, the doctors' council decided to perform external cephalic rotation. The manipulation was performed in the operating room, with full readiness for an urgent cesarean section, which meets modern safety standards.
Thanks to the coordinated work of the team, the rotation was successful: the fetal head was clearly located above the entrance to the small pelvis, the condition of the mother and child remained satisfactory. A week later, under observation, the pregnant woman gave birth naturally: physiological delivery of a large fetus (4100 g, 53 cm) took place, the newborn received 9 points on the Apgar scale. The postpartum period passed without complications, and on the third day the mother and child were discharged home.
This clinical case once again confirmed the effectiveness and safety of external fetal rotation, as well as the high level of training and professionalism of the staff of the department and clinical bases. Performing such manipulations helps to reduce the number of surgical interventions, shortens the length of stay in the hospital and improves perinatal outcomes.
